RESUMO

The increase in life expectancy combined with greater bone fragility over the years is causing a rise in the bone fracture cases. Femur fractures are the most important due to their high mortality rate. This multidisciplinary work is carried out in this context and focuses on the experimental reproduction of human femur fractures by compression. We describe a sequence of steps supervised by orthopaedic surgeons for the correct arrangement of specimens on the system set up to perform the experiment. The device applies force by compression until the human bone is fractured. All tests performed have been monitored and evaluated from different knowledge perspectives. The results obtained have demonstrated the repeatability of the fracture type in a controlled environment as well as identifying the main features involved in this process. In addition, the fractured bones have been digitized to analyze the fracture zone to recreate and evaluate future simulations.

RESUMO

Recent progress in laser wakefield acceleration has led to the emergence of a new generation of electron and X-ray sources that may have enormous benefits for ultrafast science. These novel sources promise to become indispensable tools for the investigation of structural dynamics on the femtosecond time scale, with spatial resolution on the atomic scale. Here, we demonstrate the use of laser-wakefield-accelerated electron bunches for time-resolved electron diffraction measurements of the structural dynamics of single-crystal silicon nano-membranes pumped by an ultrafast laser pulse. In our proof-of-concept study, we resolve the silicon lattice dynamics on a picosecond time scale by deflecting the momentum-time correlated electrons in the diffraction peaks with a static magnetic field to obtain the time-dependent diffraction efficiency. Further improvements may lead to femtosecond temporal resolution, with negligible pump-probe jitter being possible with future laser-wakefield-accelerator ultrafast-electron-diffraction schemes.

This study aimed to analyze the chemical composition and the IgG concentration of the colostrum, transitional milk, and mature milk of Santa Inês ewes as well as the transfer of passive immunity to lambs. Thirty-two pregnant ewes and 38 lambs were used. Ewes were milked immediately after lambing and at 12, 24, 36 h and 10 d postpartum. Colostrum was provided to the lambs at 40±15 min (mean±SE) after birth and then at 30-min intervals for obtaining the intake closest to 10% of body weight, and transitional milk was provided ad libitum. Blood from the lambs was collected 36 h after birth for measuring the serum concentrations of IgG, total protein, albumin, and gamma-globulin. The production was lower in primiparous than in multiparous ewes with body condition score (BCS)<2.75, but did not differ between primiparous and multiparous with BCS≥2.75 (interaction parity and BCS). The IgG concentration and fat, protein, lactose, and defatted dry extract percentages were not affected by the BCS of the ewe at lambing or by the parity. The total solids percentage in the colostrum was higher in ewes with BCS<2.75 (interaction BCS and time). The production and the protein, total solid, and defatted dry extract percentages showed quadratic behavior, the fat percentage decreased linearly, and the lactose percentage increased linearly with time postpartum. The IgG concentration in the colostrum was not correlated with the ewe's weight or BCS at the time of lambing. Moreover, the parity, the BCS, the ewe's type of gestation, and the lamb's sex did not influence the serum concentrations of IgG, total protein, albumin, and gamma-globulin in lambs. Adequate passive immune transfer (PIT) was observed in lambs for which the IgG intake was higher than 30 g. Failure in PIT was observed in 39.5% of lambs when considering a serum IgG concentration lower than 15 mg/mL and in 21% when considering a serum total protein concentration lower than 45 mg/mL. The mean apparent efficiency of absorption was 38.10%, with values between 0.02% and 98.80%. The serum IgG concentration was correlated with the total protein concentration (according to the enzymatic colorimetric method), the gamma-globulin concentration, and the absorption efficiency. The extreme variation on apparent efficiency of absorption may have an effect on the success of PIT. Lambs should consume at least 30 g of IgG in the first 24 h of life to ensure adequate PIT.

The effects of nursing regimens on the body condition, onset of ovarian cyclicity postpartum and weaning weight of lambs were assessed in Santa Ines ewes. Thirty-two ewes were blocked according to parity, number of lambs, and body weight at lambing and within each block randomly allocated to treatments: continuous nursing (CN), controlled nursing (CN2) with two daily feedings for an hour after the 10th day postpartum, or early weaning (EW) with total separation from the lambs after the 10th day. The animals were evaluated from the 12th day postpartum until the first estrus or until 60th day. The dry matter and nutrients intake did not differ among treatments (P>0.05) but did differ over time (P<0.01). The weight, body condition score, serum concentrations of non-esterified fatty acids and prolactin, the percentages of ewes in estrus, of ewes that ovulated within 60th day and had ovulation silent, the period from lambing to estrus, ovulation and follicle with a diameter ≥5mm and the maximum follicular diameter did not differ (P>0.05) among the treatments. The percentage of ovulation until 30th day was greater (P<0.05) in the EW group. The percentage of short luteal phases was higher in the CN2 and EW groups (P=0.07) and normal luteal phases were higher in the CN group (P=0.01). Lamb weight weaning was lower in the EW group (P<0.05). It is possible to use CN to obtain lambing periods less than eight months in Santa Ines ewes, with the advantages of simpler management and higher lamb weaning weights.

The use of tensilely strained Ge nanomembranes as mid-infrared optical gain media is investigated. Biaxial tensile strain in Ge has the effect of lowering the direct energy bandgap relative to the fundamental indirect one, thereby increasing the internal quantum efficiency for light emission and allowing for the formation of population inversion, until at a strain of about 1.9% Ge is even converted into a direct-bandgap material. Gain calculations are presented showing that, already at strain levels of about 1.4% and above, Ge films can provide optical gain in the technologically important 2.1-2.5 µm spectral region, with transparency carrier densities that can be readily achieved under realistic pumping conditions. Mechanically stressed Ge nanomembranes capable of accommodating the required strain levels are developed and used to demonstrate strong strain-enhanced photoluminescence. A detailed analysis of the high-strain emission spectra also demonstrates that the nanomembranes can be pumped above transparency, and confirms the prediction that biaxial-strain levels in excess of only 1.4% are required to obtain significant population inversion.

Quarenta e três ovelhas foram distribuídas em quatro tratamentos em um delineamento de blocos casualizados. Os tratamentos consistiram no fornecimento de dietas de flushing, formuladas com farelo de soja (FFS) ou com glúten de milho e farelo de algodão (FGM+FA) por 28 dias antes da ovulação, seguido pelo fornecimento de dietas de flushing ou de uma dieta de mantença (DM) durante 28 dias após a ovulação: T1 - FFS antes e após a ovulação (n=12); T2 - FFS antes da ovulação e DM após (n=11); T3 - FGM+FA antes e após a ovulação (n=10); T4 - FGM+FA antes da ovulação e DM após (n=10). O ganho de peso e o peso final não diferiram (P>0,05) entre os animais dos tratamentos. O ECC final foi maior (P<0,05) nas ovelhas do tratamento T3 do que nas do tratamento T2. A concentração de N-ureico foi maior (P<0,05) nas ovelhas dos tratamentos T1 e T3 do que naquelas dos tratamentos T2 e T4. A taxa de gestação e a prolificidade não diferiram (P>0,05) entre os animais. As dietas de flushing antes da ovulação, seguidas pelo fornecimento destas mesmas dietas ou de uma dieta de mantença após a ovulação, não alteraram a taxa de gestação e a prolificidade.


Forty-tree ewes were assigned to four treatments in a randomized block design. The treatments consisted of two flushing diets, composed of soybean meal (SMF) or corn gluten and cottonseed meal (CG+CMF) furnished for 28 days before ovulation, and feeding with flushing diets or a maintenance diet (MD) for 28 days after ovulation: T1 - (SMF) before and after ovulation (n=12); T2 - SMF before ovulation and a MD after (n=11); T3 - CG+CMF before and after ovulation (n=10); T4 - CG+CMF before ovulation and MD after (n=10). The gain in BW and the final BW did not differ (P>0,05) between treatments. The final BC was higher (P>0,05) in ewes in T3 treatment than in those in treatment T2. The urea N concentration was higher (P<0,05) in ewes in treatments T1 and T3 than those in treatments T2 and T4. The pregnancy rate and prolificacy did not differ (P>0,05) between treatments. The flushing diets before ovulation and feeding with this same diet or a maintenance diet after ovulation did not alter the pregnancy rate and prolificacy.

BACKGROUND: Bone metastases are common in patients with advanced non-small-cell lung cancer (NSCLC) and can have devastating consequences. Preventing or delaying bone metastases may improve outcomes. PATIENTS AND METHODS: This study evaluated whether zoledronic acid (ZOL) delayed disease progression or recurrence in patients with controlled stage IIIA/B NSCLC after first-line therapy. Patients received vitamin D and calcium supplementation and were randomized to i.v. ZOL (every 3-4 weeks) or no treatment (control). The primary end point was progression-free survival (PFS). RESULTS: No significant intergroup differences were observed in PFS or overall survival (OS). Median PFS was 9.0 months with ZOL versus 11.3 months for control. Fifteen ZOL-treated (6.6%) and 19 control patients (9.0%) developed bone metastases. Estimated 1-year OS was 81.8% for each group. ZOL safety profile was consistent with previous clinical data, but with higher discontinuations versus control. Fifteen ZOL-treated (6.6%) and five control patients (2.3%) had renal adverse events. Two cases of osteonecrosis of the jaw were reported. CONCLUSIONS: ZOL did not significantly affect PFS or OS in stage IIIA/B NSCLC patients with controlled disease, with a trend toward worsening PFS in the longer-term follow-up. Few patients experienced bone metastases, possibly limiting the potential ZOL impact on disease course.

Objetivo. Evaluar la validez diagnóstica de la tomografía por emisión de positrones con 18F-fluoro-2-deoxi-D-glucosa y la tomografía computarizada (PET/TAC) en la estadificación ganglionar mediastínica (N2) en pacientes con cáncer de pulmón de células no pequeñas (CPCNP) y analizar el papel de la estadificación mediante pruebas invasivas para la verificación de los hallazgos de la tomografía por emisión de positrones (PET)/tomografía computerizada (TAC). Material y métodos. Estudio retrospectivo incluyendo a pacientes con diagnóstico histopatológico de CPCNP, estadificados como N2 mediante TAC+C y estudiados con PET/TAC. Se confirmaron mediante análisis histopatológico de la pieza quirúrgica cuando se dispuso o por consenso iconográfico en el comité de tumores torácicos. Se ha calculado la sensibilidad, especificidad, valor predictivo positivo y valor predictivo negativo del estudio PET/TAC en la correcta clasificación de los pacientes N2. Resultados. Se incluyeron 34 pacientes clasificados como N2 mediante TAC+C. El PET/TAC clasificó a 30 pacientes como estadio N2. Se encontraron discrepancias en 4 pacientes, dos clasificados como N1 y dos como N0. Los resultados fueron confirmados histológicamente en 20 pacientes. El estudio PET/TAC no mostró falsos positivos. La sensibilidad fue del 94,7%, la especificidad y el valor predictivo positivo fue del 100% y el valor predictivo negativo del 50%. Conclusiones. El estudio PET/TAC presenta una alta sensibilidad y valor predictivo positivo en la correcta clasificación de pacientes con afectación ganglionar mediastínica (N2). Nuestros resultados sugieren que en pacientes con cáncer de pulmón potencialmente resecable, candidatos a tratamiento con quimioterapia neoadyuvante, la mediastinoscopia podría reservarse para la re-estadificación(AU)

Avaliou-se o efeito da adição de soro de leite líquido à dieta sobre os parâmetros sanguíneos e sobre a digestibilidade aparente da matéria seca (DAMS), da proteína bruta (DAPB), da fibra em detergente neutro (DAFDN) e da fibra em detergente ácido (DAFDA) em 12 vacas Girolando, secas, que receberam feno de coastcross (Cynodon dactylon), suplementado com sal proteinado, e zero (controle), 15, 30 ou 45 litros de soro de leite/dia. A adição de soro na dieta afetou a DAMS e a DAPB (P<0,01) e não houve efeito sobre a DAFDN e a DAFDA. Quanto maior a quantidade de soro, maior o valor da DAMS e da DAPB. Os valores médios de glicose no plasma sanguíneo - 59,3; 64,0; 66,6 e 69.2mg/dL - variaram (P<0,01) entre os tratamentos, ajustando-se a regressões lineares positivas. A inclusão de soro afetou (P<0,01) a quantidade de ureia no sangue de maneira decrescente - 22,83; 20,17; 17,5 e 15,67. O soro de leite melhorou a eficiência de utilização de compostos nitrogenados no rúmen e pode ser utilizado para complementar suplementos proteicos com elevados teores de ureia.


The effect of liquid whey addition in the diet on blood parameters and on the apparent digestibility of dry matter (ADDM), crude protein (ADCP), neutral detergent fiber (ADNDF), and acid detergent fiber (ADADF) was evaluated in 12 dry Gir cows, receiving coastcross (Cynodon dactylon) hay supplemented with protein salt and zero (control), 15, 30, or 45 liters of whey per day. The inclusion of the whey in the diet affected the ADDM and ADCP (P<0.01) and had no effect on ADNDF and ADADF. As high the volume of whey inclusion, higher the ADDM and ADCP values. The average values of glucose in blood plasma - 59.3, 64.0, 66.6, and 69.2mg/dL - varied (P<0.01) among treatments, adjusting themselves to positive linear dL regressions. The whey inclusion diminished (P<0.01) blood urea values - 22.83, 20.17, 17.5, and 15.67. The whey improved the efficiency of utilization of nitrogen compounds in the rumen and can be used to complement protein supplements with high levels of urea.

Estudou-se a influência da energia metabolizável (EM), pela inclusão de percentuais crescentes de fibra em detergente neutro (FDNf), proveniente de forragem, sobre a composição dos tecidos da carcaça de 64 cordeiros Santa Inês. Os tratamentos (dietas) foram: dieta A - com 8,7 por cento de FDNf; dieta B - com 17,3 por cento; dieta C - com 26,0 por cento e dieta D - com 34,7 por cento de FDNf na dieta. Quatro animais de cada tratamento foram abatidos aos 43, 83, 123 e 173 dias. Os cordeiros que receberam as dietas A e B apresentaram maiores pesos dos tecidos ósseo (TO), muscular (TM), adiposo (TA) e outros (OT) na carcaça fria, a partir dos 83 dias de idade. O rendimento de TO na carcaça fria dos animais que receberam as dietas A e B foi menor, enquanto as proporções de TA foram maiores a partir dos 83 dias de idade. A regressão de cada variável, em função do consumo de EM total (Mcal), para cada tratamento, mostrou que a deposição de TA em cordeiros alimentados com a dieta A foi mais intensa.


The effect of metabolizable energy level (ME), due to increasing levels of forage nitrogen detergent fiber (fNGF), on the carcass tecidual composition of 64 Santa Inês lambs was evaluated. Four treatments (diets) were: diet A- fNDF; diet B- 17.3 percent; of fNDF; diet C- 26.0 percent of fNDF; and diet D- 34.7 percent of 8.7 percent fNDF in the diet. Four animals of each treatment were slaughtered at 43, 83, 123, and 173 days. Lambs fed diets A and B, showed higher weight of bonne tissue (BT), muscular tissue (MT), fatty tissue (FT), and other tissues (OT) in cold carcass at 83 days of age. The percentage of BT in cold carcass of the lambs fed diets A and B was lower, and percentage of FT was higher, after 83 days of age. The regression of each variable on ME total intake (Mcal), for each treatment, suggested higher intensity of deposition of FT for lambs fed diet A.

INTRODUCTION: Bioimmunochemotherapy (BCT) is a combination of biological agents and cytostatics that has shown an increase in response rate (RR) in metastatic melanoma patients. The aim of the study is to evaluate RR, progression- free survival (PFS), overall survival (OS) and treatment toxicity. MATERIALS AND METHODS: Retrospective analysis of 11 metastatic melanoma patients treated from January 2002 to June 2008 with cisplatin 20 mg/m(2) i.v. days 1.4, dacarbazine 800 mg/m(2) i.v. day 1, vinblastine 1.5 mg/m(2) i.v. days 1.4, interleukin (IL)-2 9 MIU/m(2) s.c. 5.8 days and interferon (IFN)-alpha-2b 5 MIU/m2 s.c. days 5.9, 11, 13 and 15, with the support of granulocyte colony-stimulating factor (G-CSF) and antibiotics. Patients with ECOG 0, age < or = 65 years and with measurable disease were included. The planned number of courses was 4. RR was measured by Revised Evaluation Criteria in Solid Tumour (RECIST) criteria (computed tomography [CT]+/-proton emission tomography [PET]). Toxicity was measured according to the National Cancer Institute (NCI) common toxicity criteria. RESULTS: Observed RRs were 18% complete response (CR), 27% partial response (PR), 9% stable disease (SD) and 46% disease progression. The median PFS was 4 months (95% CI, 0.10 m), with a 23% one-year PFS. Median OS was 4.6 months (95% CI, 0.9.19 m), with a 29% one-year OS. Eighty-three percent of patients experienced grade 3-4 toxicity, mainly due to neutropenia, thrombocytopenia and flu-like syndrome. CONCLUSIONS: Treatment with BCT shows an increase in RR, some achieving durable CR; nevertheless it cannot be considered a standard treatment and should be employed only in selected patients (AU)

Avaliaram-se o consumo, a digestibilidade aparente e o comportamento ingestivo de ovelhas no terço final de gestação com dietas contendo diferentes níveis de fibra em detergente neutro forrageiro (FDNf). Foram utilizadas 16 ovelhas adultas da raça Santa Inês, distribuídas em um delineamento inteiramente ao acaso, com quatro tratamentos: 9, 17, 26 e 35 por cento de FDNf. Os consumos de matéria seca, matéria orgânica, proteína bruta, energia bruta, energia digestível e energia metabolizável não foram influenciados pelos tratamentos, e os consumos de fibra em detergente neutro (FDN) e de fibra em detergente ácido (FDA) aumentaram com maiores níveis de FDNf. As digestibilidades do FDN e FDA e o tempo gasto pelos animais com ruminação também aumentaram com maiores níveis de FDNf na dieta. Dietas com maior quantidade de FDNf influenciaram comportamento ingestivo e digestibilidade de ovelhas em fase final de lactação.


The intake, apparent digestibility and feeding behavior of pregnant ewes ped diets containing different levels of neutral detergent fiber (NDF) were evaluated. Sixteen Santa Inês ewes were randomly assigned to four treatments with 9, 17, 26 e 35 percent of NDF. Dry matter (DM), organic matter, crude protein, crude energy, digestible energy and metabolizable energy intake were not affected by the level of NDF in the diet, however, NDF and acid detergent fiber (ADF) intake increased with the increase of NDF diet levels. The apparent digestibility of the NDF and ADF and rumination time increased with the NDF diet level. Higher NDF diet levels influenced feeding behavior and apparent digestibility of ewes at the end of the lactation period.

Avaliou-se a evolução do peso testicular de cordeiros Santa Inês, alimentados com diferentes níveis de energia. Foram utilizados 64 cordeiros, distribuídos em quatro tratamentos: A - 8,7 por cento; B - 17,3 por cento; C - 26,0 por cento e D - 34,7 por cento de fibra em detergente neutro (FDN), proveniente da forragem na dieta, determinando a variação no consumo de energia metabolizável. Quatro animais de cada tratamento foram abatidos nas idades pré-determinadas de 43, 83, 123 e 173 dias de idade. Os testículos foram separados dos respectivos epidídimos e pesados separadamente. Os animais que receberam as dietas A e B foram os que apresentaram maior consumo de energia metabolizável (14,11Mcal/PV0,75), os mais pesados (18,89kg e 17,09kg, respectivamente) e os de maiores pesos dos testiculos (62,54g e 27,16g, respectivamente), indicando que o desenvolvimento testicular é altamente dependente do desenvolvimento corporal e da quantidade de energia metabolizável consumida. A predição do peso dos testículos por meio da circunferência escrotal mostrou ser mais eficiente do que por meio da idade e do peso vivo dos animais.


The development of the testicule weight of Santa Inês lambs, fed different energy levels, was evaluated. Sixty-four lambs were distributed in four treatments: A - 8.7 percent; B - 17.3 percent; C - 26.0 percent and D - 34.7 percent of neutral detergent fiber (NDF) supplied by the diet forage, determining the variation of the metabolizable energy intake. Four animals from each treatment were slaughtered at pre-defined ages of 43 days, 83 days, 123 days and 173 days. After slaughtering, the testicles were separated from the epididimous and individually weighted. The animals fed diets A and B presented higher metabolizable energy intake (14.11Mcal/LW0.75), higher live weight (18.89kg and 17.09kg, respectively) and higher testicules weight (62.54g and 27.16g, respectively), indicating that the testicular development is highly dependent on the body development and the quantity of metabolizable energy intake. The prediction of the testicules weight by the scrotal circumference showed to be more efficient than the age or the live weight of the animals.

Avaliaram-se os efeitos da restrição alimentar pré e pós-natal sobre o crescimento dos órgãos internos de cordeiros Santa Inês. Foram utilizados 68 cordeiros distribuídos em três tratamentos: restrição pré-natal (RPRE), restrição pós-natal (RPOS) e sem restrição (controle). Os animais foram abatidos ao nascimento e aos 10, 15, 25, 35 e 45kg de peso vivo. Após o abate, coração, pulmões, traquéia/esôfago, fígado, baço, pâncreas, rúmen/retículo, abomaso, omaso, intestino delgado e grosso foram retirados e pesados separadamente. A análise dos pesos dos órgãos internos dos animais abatidos ao nascimento demonstrou que a restrição pré-natal diminuiu os pesos do coração, pulmões, traquéia/esôfago, abomaso, pancrêas e baço, com médias de 54g, 16g, 22g, 16g, 2g e 5g, respectivamente. Aos 15kg, o peso dos órgãos dos animais submetidos à restrição pré-natal não diferiu dos demais, indicando crescimento compensatório desses órgãos já na fase de aleitamento. Não houve efeito da restrição sobre o peso médio dos órgãos da cavidade torácica, e na cavidade abdominal o rúmen, o intestino delgado e o fígado tiveram seus pesos reduzidos, com médias de 385g, 488g e 371g, respectivamente. Pulmões, traquéia e coração apresentaram crescimento heteregônico negativo (b<1); para a traquéia e o coração dos animais RPRE o comportamento foi isogônico (b=1). O rúmen e o omaso apresentaram crescimento heterogônico positivo (b>1), o abomaso heterogônico negativo (b<1), e os intestinos, o baço e o fígado, isogônico (b=1). O pâncreas dos animais controle e RPOS apresentou crescimento isogônico (b=1), e o dos RPRE heterogônico positivo (b>1). A restrição alimentar pré e pós-natal afetou o tamanho, as taxas de crescimento e o tipo de desenvolvimento de vários órgãos internos de cordeiros Santa Inês, evidenciando a necessidade de suplementação adequada para as fêmeas no terço final da gestação e para os cordeiros, durante a fase de crescimento.


The effects of pre and postnatal nutritional restriction on allometric growth of thoracic and abdominal organs of Santa Inês lambs were evaluated. Sixty-eight lambs were divided into three groups: lambs with prenatal nutritional restriction (PRER), lambs with postnatal nutritional restriction (POSR), and lambs without restriction (control). The lambs in each group were slaughted at birth, 10kg, 15kg, 25kg, 35kg, and 45kg of live weight. After slaughtering, heart, lungs, trachea and esophagus, liver, spleen, pancreas, rumen-reticulum, abomasum, omasum, and small and large intestines were individually weighted. At birth, PRER affected the weights of heart, lungs, trachea and esophagus, abomasum, pancreas, and spleen, with means of 54g, 16g, 22g, 16g, 2g, and 5g, respectively. However, the organs of the PRER lambs at 15kg of live weight showed the same weight of that organs of the others animals, indicating a compensatory growth. POSR did not affect the weight of the organs of thoracic cavity;however, in abdomen cavity, rumen, small intestine, and liver were affected, with means of 385g, 488g, and 371g, respectively. The allometric study showed that rumen and omasum presented a positive heterogonic development; while abomasum presented a negative heterogonic development; and liver, small and large intestines showed an isogonic development. The pancreas of control and POSR lambs presented an isogonic development, while pancreas of PRER lambs showed a positive heterogonic delevopment. In this experiment, the alimentary restriction affected weigth, growth, and type of development of internal organs of Santa Inês lambs, evidencing the importance of the feeding for ewes at the last third of gestation as well as for the lambs during the growth phase.

Avaliaram-se os efeitos da restrição pré e pós-natal sobre o tamanho e o crescimento dos depósitos de gordura de cordeiros Santa Inês. Foram utilizados 68 cordeiros, distribuídos em três tratamentos: com restrição pré-natal (RPRE), com restrição pós-natal (RPOS) e sem restrição (controle). Dentro de cada tratamento, os animais foram aleatoriamente sorteados para serem abatidos após o nascimento e com 10, 15, 25, 35 e 45kg de peso vivo. Os depósitos de gordura (omental, mesentérica, perirenal, pélvica e inguinal) foram registrados. Ao nascimento, os depósitos de gordura foram menores nos animais do tratamento RPRE. Entre 15 a 35kg de peso, a quantidade de gordura foi igual nos animais dos três tratamentos e, a partir desse peso (35kg), com exceção da gordura pélvica, os depósitos no animais do tratamento RPRE foram significativamente maiores. A restrição pré-natal elevou consideravelmente as taxas de ganho da gordura omental, mesentérica, perirenal e pélvica, enquanto a restrição pós-natal influenciou somente a taxa de ganho da gordura inguinal. Cordeiros de todos os grupos apresentaram taxas de desenvolvimento tardias (b>1).

Lambs of each group were randomly sampled and slaughtered at birth and weighing 10, 15, 25, 35 and 45kg. Fat depositts (omental, mesenteric, perirenal, pelvic and inguinal) of slaughtered lambs were recorded. At birth fat deposits were small for animals from treatment RPRE. From 15 to 35kg of live weight the amount of fat was similar for animals from all groups but lambs from PRER group, with live body weight heavier than 35kg, showed higher fat deposits except for pelvic fat. Prenatal nutritional restriction increased increased the omental, mesenteric, perirenal, and pelvic fat deposits and postnatal restriction decreased the inguinal fat deposits. Lambs from all groups showed a positive heterogenic development (b>1).

BACKGROUND: Emergency department short-stay units (EDSSUs) are currently emerging worldwide as an alternative to standard inpatient hospitalisation. In our hospital, a 960-bed teaching tertiary institution in Barcelona, Spain, an EDSSU has been in operation during winter periods (November-March) since 1997. AIM: To determine the efficacy and safety of our EDSSU. METHODS: Retrospective analysis of activity and quality outcomes, assessment of patient satisfaction levels and determination of the diagnostic-related groups that were mainly responsible for admissions to the EDSSU, comparing the clinical characteristics of those patients with the characteristics of patients with similar clinical diagnoses admitted to standard hospitalisation units. RESULTS: 5666 patients were treated in the EDSSU, with a progressive increase in the number of patients admitted per period, ranging from 707 in 1997-8 to 1227 in 2003-4 (73.5% increase). The mean length of stay ranged from 3.1 to 2.8 days, mortality from 2.5% to 5.1%, home discharge rate from 84% to 90%, and hospital readmission rate within the first week after discharge from 3.9% to 6.2%. In all, 98% of patients were satisfied with their stay at the EDSSU. The main diagnostic-related groups were chronic obstructive pulmonary disease (COPD = 50%) and acute heart failure (28%). Patients with COPD admitted at the EDSSU (n = 545) showed significantly (p = 0.05) lower mean length of stay (3.4 v 12 days) and mortality (1.7% v 8.1%), but a higher hospital readmission rate (9.9% v 7%) than those admitted to standard inpatient units (n = 1961). CONCLUSIONS: In our experience, the EDSSU proved to be an effective and safe alternative to standard inpatient hospitalisation.

This study was designed to assess the viability and fecundity of semen stored at 5 degrees C for 24 hours using the Bio-Tranz shipping system. Semen specimens were assessed for motility and sperm membrane integrity at the time of collection and 24 hours after storage in the Bio-Tranz. In group 1 (n = 61), specimens were diluted in TYB, processed and used for intrauterine insemination (IUI), leaving an aliquot for storage for 24 hours in the Bio-Tranz. In group 2 (n = 67), specimens were diluted in TYB, stored for 24 hours in the Bio-Tranz and then processed and used for IUI. In both groups, the total motile sperm used for IUI was similar and the women that underwent IUI were standardized for ovulation prediction and time of insemination. The overall sperm characteristics between the two groups were within normal range. Significant decreases were noted in sperm motility and membrane integrity in both groups after storage. Similar pregnancy rates were obtained between the two patient populations. The use of the Bio-Tranz shipper is extremely convenient for patients requiring semen evaluation, cryostorage or IUI and other assisted reproductive technologies.

SETTING: The epidemiology of tuberculosis (TB) in urban populations is changing. Combining conventional epidemiological techniques with DNA fingerprinting of Mycobacterium tuberculosis can improve our understanding of how TB is transmitted. OBJECTIVE: To improve the definition of molecular epidemiology of TB over 10 years in an area of Europe not previously studied. DESIGN: A population-based retrospective study was conducted in the Autonomous Community of Madrid, Spain, from 1992 to 1998; from 1999 to 2001, the study was prospective. The study population consisted of all patients for whom positive culture and full clinical and demographic data were available. All strains were typed by RFLP. Non-clustered patients were compared with clustered patients and studied using univariate analysis and a logistic regression model. RESULTS: Of 448 patients studied, 228 (50.7%) were clustered. Youth was the strongest risk factor associated with clustering. Pleural effusion was also found to be associated with clustering. An epidemiological link was found in only 85 (37.4%) of the 228 patients belonging to a cluster. CONCLUSION: Youth and pleural effusion were identified as risk factors for clustering. These findings may help adjust TB control and contact tracing strategies.
